Making an Appointment |
Our office is open Monday through Friday, 8:30
a.m. to 5 p.m. To schedule an
appointment with one of our doctors, call (210) 614-8612. When
you call, please
have your insurance information (if any) and referral number (if you
are a
member of an HMO or PPO) available. New patients must obtain a
referral for an initial office
visit. Most insurance companies require us to verify your
coverage, deductible, and/or co-
insurance amounts before treatment. This process can be
completed prior to your appointment.
On your first visit, we ask that you arrive 45 minutes early in
order to fill out and
sign various forms which are used to create a permanent medical
record for you and have
necessary lab work completed before your appointed time with the
doctor. On subsequent visits,
please arrive 30 minutes early to allow time to update your file and
have labs drawn prior to your
physician appointment. You will not see the physician until
your lab work has been completed.
At every visit we ask that you bring the medication bottles for all
of the
prescriptions you are taking. This is necessary to insure we
do not prescribe a
medication that would interfere with those ordered by another
physician.
